The North Safety "Danger Equipment Lockout - Locked Out Do Not Operate" styrene tag is made of 20 mil polystyrene for water resistance, has lined areas for customizing with task-specific details on both sides, and has a black and white "Danger Equipment Lockout" logo and diagonal red and white stripes for high visibility. Text on the front of the tag reads, "Locked Out Do Not Operate. This lock/tag may only be removed by:" followed by three blank lined areas labeled "Name," "Dept.," and "Expected Completion." Text on the back of the tag reads, "The energy source has been LOCKED OUT. Only the individual who signed the reverse side may remove this lock/tag. Remarks:" followed by several blank lined areas. The tag includes a 1/4" diameter hole and an 8" one-piece nylon cable tie for attachment. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair. The tag measures 5-3/4 x 3 inches (146 x 76 mm) (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
Lockout/tagout (LOTO) safety procedures require that tagout devices meet Occupational Safety and Health Administration (OSHA) 1910.147(c)(5) standards for quality assurance. Lockout tags (also called safety tags) use color and warning text to help prevent machine operation or power activation during maintenance and repair. They are commonly used to identify the schedule and authority responsible for a lockout procedure. Sometimes a tag's thickness will be measured in mils. One mil equals one thousandth of an inch.
