The North Safety "Danger Equipment Lockout - A Life Is On The Line" styrene tag is made of 20 mil polystyrene for water resistance, has areas for customizing with task-specific details on both sides, and has a black and white "Danger Equipment Lockout" logo and diagonal red and white stripes for high visibility. Text on the front of the tag reads, "A Life Is On The Line. This tag and lock to be removed only by person name shown on back." Text on the back of the tag reads, "The energy source has been LOCKED OUT. Unauthorized removal of this lock/tag may result in immediate discharge. Remarks:" followed by several blank lined areas. The tag includes a 1/4" diameter hole and an 8" one-piece nylon cable tie for attachment. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair. The tag measures 5-3/4 x 3 inches (146 x 76 mm) (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
