The Accuform Signs Tags By-The-Roll lockout tag is made of 10 mil flexible plastic-based cardstock (PF-Cardstock) for water resistance, has a red, black, and white "Danger" logo on both sides and diagonal red and white stripes on the front for high visibility, and has blank lined areas for writing in task-specific details, providing the ability to attach a customized lockout warning at a specific location to help improve workplace safety. This tag comes in a roll, connected end-to-end on perforated lines for manual separation, in a portable, disposable dispenser. The 7-1/4 x 7-1/4 x 3-1/2" dispenser is a red and black octagonal cardboard box for high visibility. The tag warns of possible dismissal for unauthorized removal and can be written on to add notes and identify the persons responsible and expected completion date. It includes a 3/8" diameter hole for fastening. This tag is part of a lockout/tagout (LOTO) safety procedure required at facilities where a device or energy source needs to be isolated during maintenance and repair. The tag measures 6-1/4 x 3 inches (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
Lockout/tagout (LOTO) safety procedures require that tagout devices meet Occupational Safety and Health Administration (OSHA) 1910.147(c)(5) standards for quality assurance. Lockout tags (also called safety tags) use color and warning text to help prevent machine operation or power activation during maintenance and repair. They are commonly used to identify the schedule and authority responsible for a lockout procedure. Sometimes a tag's thickness will be measured in mils. One mil equals one thousandth of an inch.
Accuform Signs manufactures facility safety identification products. The company, founded in 1976, is headquarted in Brooksville, FL.
